Arbor Family Medicine, PC is committed to protecting the privacy of visitors to our website, as well as our clients and customers. Any information given to Arbor Family Medicine, PC, through this website will be used strictly by Arbor Family Medicine and its staff members. Volunteered information may be stored by us, but will not be disclosed to any outside entity. We will not give, sell, rent or exchange the information with anyone else without your prior consent.

What is Personal Data?

Personal data is any information relating to an identified or identifiable natural person. Standard website tracking via server logs and website analytics is not identifiable, and therefore not personal data. Information submitted via a contact form is, as name and email identity a person.

Server Logs

Our servers may record the numerical Internet Protocol (IP) address of the computer you are using; the browser software you use and your operating system; the date and time you access our site; and the Internet address of the website from which you linked directly to our site. We do not connect this information to your name or identity. We use this information to measure the number of visitors to different sections of the site, to diagnose and correct system errors, and to improve the site. We delete this information after three months.

Cookies

Our website uses cookies, but only to maintain data required for the current session. This does not include any personal data, but does include your IP address. The cookies automatically collect this data, and expire at the end of each web session.

A cookie is a tiny text file stored on your computer. Cookies store data that is used to help make sites work. Only we can access the cookies created by our website. Analytics

Google Analytics automatically tracks general visit and pageview data, including IP addresses, which is non-identifiable. We use Google Analytics to measure traffic on our website. If you’d like to opt out of tracking by Google Analytics, visit the Google Analytics opt-out page at https://tools.google.com/dlpage/gaoptout. Google Analytics is also tracking demographic and affinity data via advertising cookies, which is considered personal data. This is only used for web analytics purposes. Standard Google Analytics tracking is retained indefinitely. Demographic and affinity data from Google Analytics will be deleted 26 months after it is collected.

Email Newsletter

Name and email given to us may be shared with email vendor software to send an email newsletter. A separate option be will given to consent to receiving this newsletter on our website.

Embedded content from other websites

Content on this site may include embedded content (e.g. videos, images, articles, etc.). Embedded content from other websites behaves in the exact same way as if the visitor has visited the other website. These websites may collect data about you, use cookies, embed additional third-party tracking, and monitor your interaction with that embedded content, including tracing your interaction with the embedded content if you have an account and are logged in to that website.

Online services accessible from our website

We partner with online services to provide better communication and information to our patients. Each of them is HIPAA-compliant and has their own privacy policies.

Arbor Family Medicine uses an Electronic Health Record system called Allscripts Professional EHR where medical records are stored, as well as a Practice Management system called Allscripts PM, which handles insurance, billing, and payment records. We have both internal staff and independently contracted associates who help us in management of these massive databases. All of these individuals are required to respect and comply with government Privacy rules and have signed agreements with Arbor to do just that. Our Patient Portal, known as FollowMyHealth, provides patient-level access to these databases. Their privacy policy is at https://www.followmyhealth.com/UseDocuments/PrivacyPolicy.

Phreesia provides online bill pay services to our patients, and also manages our tablet registration system. Their privacy policy is at https://patient.phreesia.com/privacy.html.

QueueDr is a web-based platform that automatically matches patients with available appointments because of cancellations, reschedules no-shows and rebooks bumped appointments via text. Their privacy policy is at https://docs.google.com/document/d/1baWwX8BWBqAFnRdtf0Q3gP_RyTwIeHoH_R_2Cfjdpq0/edit.

DataFile is a third-party vendor who provides HIPAA compliant, accurate and timely distribution of medical records. Their privacy policy information is at https://www.datafiletechnologies.com/solutions/medical-records/hipaa-compliance/.

Accounting

We save your billing information and other information gathered about you for as long as needed for accounting purposes or other obligations deriving from law.

Children

We keep medical data, billing data, and family and Guarantor information, as required for the purposes of providing medical care and billing for services provided.

How long we retain your data

We follow standards set by the Colorado Department of Public Health and Environment. See sections 8.102-4 on pages 17-20 of Standards for Hospitals and Health Facilities.

How we protect your data

We do our best to keep your personal data safe. We use safe protocols for communication and transferring data whenever possible (such as HTTPS). We use anonymising and pseudonymising where suitable. We monitor our systems for possible vulnerabilities and attacks. Even though we try our best we cannot guarantee the security of information. However, we promise to notify suitable authorities of data breaches. We will also notify you if there is a threat to your rights or interests. We will do everything we reasonably can to prevent security breaches and to assist authorities should any breaches occur.
